The  mother is very concerned if nursing her child with an active lesion can  
transmit herpes to her nipple.
See Riordan's book for more information on another case of  transmission.  I 
knew a mother whose nipples were very painful for  over a week and another 
couple weeks to healing after her toddler got a severe  case of cold sores in her 
mouth.  Mother took acyclivor, as did her  toddler and she also used the 
anti-viral cream on her nipples.   She also added betamethasone and bactroban at 
one point "for good measure" and  thought the betamethasone did help a lot with 
the pain by reducing the  swelling.  She did not wean her toddler, but 
nursing was pretty tough for  awhile.
