Jack Newman had an "Immediate Postpartum Breastfeeding Decision Tree"  
in an article in the Journal of Human Lactation, Volume 12, Number 2,  
June 1996, pages 129-135.  The article is titled, "Decision Tree and  
Postpartum Management for Preventing Dehydration in the 'Breastfed'  
Baby."  The decision tree is pictured on page 133.  This might be  
what you are looking for.  You could adapt it for your needs.
