Thanks to those who have replied, on and off list, to my question.  I am
blind to the systematic, taken-for-granted aspects of how my workplace runs,
but I can see very clearly where some of the barriers to good BF support
arise from the way care is organized in some of the places described to me
in the US.
Failing to treat mother and baby as a unit with same staff responsible for
care of both
Having a nursery where babies stay without their mothers for whatever reason
or for however long
Having to do charting in a different room from where the mother and baby are

Once these have been addressed you can start looking at the particulars of
WHAT nurses should use their newly freed time to do to facilitate the start
of breastfeeding.
