I would not expect this to be caused by anything the Mother did or did not do as far as her health.  My guess would be a lack of functional breast tissue on the one side.  Of course you could still try low milk supply interventions, since we never assume a breast won't produce based on appearance.  However, it is quite possible to nurse a baby exclusively on one side and make plenty of milk.
