I'm just throwing out the first few things that come to mind here.
First - Baby issues? It sounds as if MOM has been looked at pretty
extensively, but what about the baby?  Perhaps a  tongue tie or other
issues?  Has someone evaluated the frenulum?  Has anyone tried craniosacral
therapy or chiropractics on the baby?

Secondly - has anyone done an evaluation for inflammatory breast cancer? 

I'm sure six other things will pop into mind as soon as I post this.  The
IBC stuck with me as I just read about the lady with IBC online who got
diagnosed from pain symptoms (the one that has a son with the breastfeeding
tattoo - loved the tatoo, the story broke my heart).
