Ok, I'll just post this quickly and then get back to topic.  After having the
pleasure of being with another US ex-pat friend when she had her first baby,
which included her reaching down and receiving her daughter into her own hands
while I stood by watching, I had to assure her that the 5 minutes she had
sampled the nitrous oxide around transition did **not** mean that she had had
a medicated birth.  Three weeks later she BIKED to town from her home.  It was
only about half a mile and no steep hills, but still.  The sight of a bike
seat would have stricken fear into my heart at that point.
Everything is relative.  This same friend breathed and hummed to the tune of
Amazing Grace as she was transitioning in labor number 2.  Try it, you'll find
the rising and falling pitch fits the pattern of those intense contractions
really well!
She is just a regular woman.  Not a deviant in any sense of the word.  She
did, however, feel unable to share with other mothers on the post partum ward
her story of how she coped at the most intense part of labor.  :-)
