This makes absolutely no sense.  There is tons of documentation that 
breastmilk is best especially for preterm infants.  You and the mother 
must be so frustrated with the doctor's insistence on only formula for 
this baby.  If you are in the US, the mom can legally insist that it is 
her baby and the baby is to be given her milk, with fortifier if 
neccessary.  She needs to be careful that she remains calm and 
reasonable, so the staff does not fight back by ordering a psych consult 
for her.

The calculation you are looking for is 2.5 oz milk per lb body weight 
per day, but it refers to full term infants.  There are various 
calculations for premies, based on numbers of ml per kg body weight per 
day.  I've read everything from 150ml and up.
