I was in contact with various flight attendants while living in Puerto Rico.  Some traveled to Europe, or to western US or South America - no matter which direction, they needed to pump while in flight, and they usually had no problem getting support from their team.  The returning team would bring the milk back to home base.  A few times they used Dry Ice (usually in hot climates) from an Ice Cream Store if they had to keep it in the baggage compartment until the next flight back.  

I only wish Breastfeeding In Combat Boots had been available for them - but it was many years ago!
