One thing I see now and again is that the baby's tongue looks tied - but a
compounding factor is some seriously tight muscles.  On more than one
occasion with this dual challenge I have sent mom to have the baby's muscles
worked on first and when they release and relax, the tongue tie issue become
moot.  There was no tie - it just looked like because the muscles in the
neck, jaw and under the chin were impeding proper tongue motion.and a suck
eval demonstrated t-t like motion.  But there were other factors that made
me not 100% convinced that it was a true t-t.



I had a mom come to my mommy group Friday - she came with BF problems.  I
looked at the baby (3 other LC's had looked prior to me) and I was the only
one who saw this baby's head firmly attached to her shoulders (where as her
neck???) due to severely tight muscles related to a traumatic birth AND she
also had a classic t-t - the frenulum was connected from the base of the
tongue to the lower gum ridge.  Why the others missed the t-t I don't know.
Mom said none of them ever looked in the baby's mouth - just worked on
positioning and latch then gave her a nipple shield (which didn't help).but
why isn't someone looking in the baby's mouth when mom complains of severe
pain with the baby is latched?  For this baby there are 2 issues - t-t and
severely tight muscles - both of which can clearly impact baby's ability to
BF successfully.  Gave her a couple referrals and will probably see her
later this week.
