Dear Wise Ones,
I have a client who has given permission to post.   She is a 1st time   mom,
baby born via c-section after SROM at 36 weeks.   C-section was planned for 38
weeks due to some ?? IUGR/SGA issues.   Baby was about 6 lbs at birth,
dropped less than 10% and started to gain normally.   The mom has somewhat flat,
large nipples normally, all the IVs made her whole body swell to the point of
pain to the touch.   She was sent home with a lactina and nipple shield and
instructions for tube/syringe feeding.   After the initial edema subsided, she
started getting engorgement at the wierdest times, like an hour after a good feed
or pump.   The Dr. DX was mastitis although there was NO fever.   The
antibiotics seemed to make a difference right away and then bammo, about 5 days into
treatment, the engorgement/pain came back again.   The pain and swelling is all
the way to both armpits.   this can happen so quickly to her, not when I
would expect it, like if she slept thru a pump or feed or the baby wasn't feeding
well, and woke up 6 hours later.   This can suddenly happen within an hour or
2 hours of feeding or pumping.   When it happens, she can barely get out any
milk with the pump and the baby struggles.   Then it will resolve in about 12
hours.... The baby does very well at the breast with the shield.   We switched
to the extra large flanges on the pump which lessend the pain with the initial
pumping.   Is there an Acute form of Mastitis that comes and goes like this
or am I missing something?   Even with massaging, heat, cold, and everything
else I can think of, she will be fine for a couple of days, think "OH boy I am
on easy street now"   only to be hit with it all over again...   It is so fru
strating for her, she has been a trooper...
any suggestions???
