Reference please!

At the ILCA conference in Scottsdale in '95 I remember hearing about a study
done in a pediatrician's waiting room.  It showed 1/2 of the bottles of
formula that mothers brought in with them were prepared incorrectly.

Who has heard about this study?

Can anyone help me find the reference?
