Try finger feeding.  Some babies like the skin-on-skin feel and after 1/2
oz or 1 oz don't feel the stress of hunger and will calm and accept a
bottle nipple.
    I helped one couple like this with a 2 month old.  The baby
completely relaxed while nursing at the end of the consult. I thought
this might be a difficult baby to watch apart from mom.   Dad was to be
primary care-giver starting the next week.  I mentioned lots of skin to
skin time with Dad and can he go to the workplace for nursing during
lunch breaks; which he could.
