I wonder if the motor is worn out? Pump in Style is rated to last about 
300 hours of pumping, and is designed to be used about 3 times a day. 
It's not engineered/designed to be the sole source of breast stimulation 
and milk removal.

The mom may be putting the vacuum up too high to try to compensate for 
the motor aging, and using the huge flanges may be allowing a lot of 
breast to get trapped in the flange, but reducing the circulation in the 
front of the breast. That would make the nipple skin stretch and more 
likely to burst from normal stresses.

Do you have a suction gauge to check the pump with? I would encourage 
the mom to use a rental pump for a few days and see if it's more 
comfortable, and see if she can step back down to the 30mm flanges.
