Pat,
Can she use an Evert-It or adapted syringe to continue to work on her
nipples?  Is she willing to put out the cost and hassle of getting
Nipplettes from England (I think that's where they come from)?  If the
nipple does evert in the shield, then I would think there's still the
possibility she might get the tissue loosened enough to stay out for
baby to latch.  On the other hand, maybe they will only evert with the
more continuous negative pressure of baby's suckling.  Whether to keep
on working on them, or to be content with nursing through shield depends
on mom's patience and tenacity.  Only you and she together can make that
judgementcall.
Incidentally, even with never having another kind of nipple in the mouth
I have seen a few infants who still have a big problem accepting the
inverted nipple.  It seems some require that stimulation to the back of
their mouth that an erect nipple gives before they will agree to suckle.
