Anne,
Along with reflux it would be worthwhile exploring possibility of
allergy/food intolerance symptoms.  Have you discussed allergy history
and allergens coming through breastmilk  (or is baby getting vitamins or
formula or anything else)?  It can't hurt and might help to have mom keep
a food and drink and vitamin etc diary.  On opposite side of page have
her record baby's behavior.  When baby has a good day, emphasize the
food/drink mother had that 24 hours while tracking down what was
different on the bad days.   It may seem strange, but even though the
milk is the same, the symptoms may be stronger when baby breastfeeds vs.
alternative feeds.  Hmmm.  As I consider that this baby likes alternative
feeding I wonder about a  nipple shield in this situation....  Would it
change the behavior or not?

Also might have a cranial-sacral therapist or physician or chiropractor
check to make sure baby doesn't have any birth injuries that are hurting
off and on.  Does baby cry in certain positions and not others?  I had
one baby do this--some good and bad feedings with "colic"  that turned
out to be from undiagnosed hip and shoulder injuries.

Hoping this mom is *seen* by someone skilled and not just talking to all
these LLLLs and LCs on the phone.
