Both my children were born at home.  My son was not immediately nurtured 
at breast because of (surprise!) huge numbers of visitors even though we 
told them not to come.  So with a bili of 25.9, we ended up in the 
hospital.  My milk came in at about 72 hrs pp.
When my daughter was born, we had an outright ban on visitors.  She 
nursed within minutes of birth, and while I never experienced 
engorgement, I clearly had quite a bit of milk within 24 hrs.
