If the nipple is creased, the latch is not "perfect", IMHO.  Try to get
the latch more asymmetrical, check the baby for tongue tie or short
mandible which can lead to compensatory excessive jaw compression, and
show mom how to manually express colostrum into a teaspoon to feed this
little baby.
