"But is there really anyone who doesn't know that? "

Yes, there really are women who either don't know this, or choose to
ignore this, or can't get themselves to a place where they can admit to
an addiction. We had a case like this last month. hospital refused to
allow mom to nurse baby because she tested + for cocaine. (Not clear when
this happened, prenatally or during labor). Mom stated she didn't take
anything, was just at friend's house while the friend was doing coke.
Mom did get to take baby home. mom pumped during baby hospitalization.

So, there are both ends of the spectrum. I think with some women, they
feel that a little bit or one time won't hurt anything. but that can be
the nature of an addiction. Maybe these women are just self-medicating.
