Lori, sounds like you and your colleague have tried everything I would
try up to this point.  Has anyone done a digital exam on the baby's
oral cavity?  This baby could be pushing up hard with the tongue in an
attempt to fill the gap due to an elevated palate.  I see this scenario
fairly regularly.  If the palate is not the issue, I would try suck
training and definitely continue the TMJ massage, progressing to an OT
evaluation if the simpler measures fail to improve or correct the
problem.
