Hello. I have a mom in our Special Care Nursery who is breastfeeding her
third child. This mother has a mass in the upper quadrant of her left
breast at about 1:00 position. It is in the breast, not axillary. This
mother reports this mass has been present with all three babies, and is
painful during feedings. There is even a clicking noise heard coming
from this spot on her breast during feedings. It is so painful to nurse
on this side, mom gave up after four months with her last child.

I'm not sure why, but baby's latch is also different on this breast.
Baby's cheeks dimple in during the feeding, as if the latch is not
correct, however with assessment, the latch does look good with proper
lip placement and seal. I do hear swallowing.

Could this be a pesistent galactocele? And what would cause the
clicking noise? I am sending mom back to her OB for evaluation and will
request an ultrasound, but am curious to know if anyone has an idea on
this.

Thanks,
Brenda Phipps, BS, IBCLC
