It is becoming more common in many specialities.
CCRN, the critical care nursing certification, also
requires it now.  Other groups are proposing
examination as a method of evaluating competency along
with documenting CEU's & practice hours.  Written
examination as well as going through hands-on
scenarios before an instructor is required each time a
medical professional is recertified for any of the
advanced life support courses...nerve racking though
it may be.  It was one of the reasons I chose to
become a pediatric advanced life support instructor.
I'd know the material inside & out to remain
up-to-date and had a lot of opportunity to facilitate
classes and find out what others across the state were
doing. The instructors also have to go through
re-examination.  I'd thought being an instructor would
decrease my fear of public speaking, but that didn't
quite do the trick.  :)
