I've encountered clients with similar symptoms several times in the past six months. I have copied this post to Chris Bertzold, who shared info with me that has been very helpful. What you describe sounds like it could be a deep ductal fungal and/or bacterial infection, which is very hard to get diagnosed and requires a longer regime of treatment than is conventially prescribed. If client can get culture of her milk done, that's helpful. Since the classic symptoms of breast infections don't necessarily present, I have found it hard to get Drs. to consider. If client has history of nipple breakdown, it seems even more likely this is the problem as that often is the way the fungus/bacteria enters the ductal tissue.
