Barbara, a couple of questions about your procedure of sending your 
assessment records and finding to a mother's primary care provider:

1.  Do you have a standard form that you use for this?  If so, could you 
share it?

2.  It is the mother that provides all the info about her care providers, 
or does she just tell you their names and do you have to track down their 
contact info?

3.  Do you provide the records to only doctors, or to midwives or other 
hcp's also?
