Does anyone have a telephone triage tool for a "day after discharge" phone
call follow-up?  I need to make a relatively short questionnaire that our
Medical Assistants could use and a more detailed one for our RN's to use
to follow-up any concerns that the MA's identify.  I would appreciate any
suggestions you might have or any tools you are using.  I'm currently
