Regarding the mother who lost 800 ml of blood post partum and is
experiencing low supply - I agree with Lisa that she may need iron, but do
check her hemoglobin and her ferritin first.

Unless the blood loss was grossly underestimated, the possibility that she
had anoxic damage to her pituitary gland is very, very remote. I may have
seen such damage once; it was 15 years ago, in a women who experienced a
uterine inversion and was transfused with approximately 1.5 times a normal
blood volume for an adult. Most healthy women tolerate a blood loss of 800
ml post partum without serious sequelae. If she were already anemic (Hb <
10) it would be a bit worse.

I see women daily who have lost a lot more blood than 800 ml and they don't
have any more problems with supply than anyone else. In fact, the women who
are so anemic that they spend the first few days in bed due to dizziness,
seem to bring in a huge supply quickly, from just lying around
breastfeeding.
