Toni, Reglan, domperidone, fenugreek, blessed thistle, etc, --all the
galactogogues-- all work to help mom increase prolactin to give her that extra boost
that makes it easier to *increase* her milk supply. Once her production is up to the
level she and her baby are content with, her baby or her pumping can take over, and
she doesn't need the herb or med.

Some people get a "placebo" effect from the medication or herb, the feeling of
fullness and the output gives them confidence, so taking the galactogogue gives them
confidence, and that aids oxytocin release, which of course makes pumping and
breastfeeding smooth.

But that same oxytocin release can be had without the galactogogue, without the
placebo effect, since these herbs and meds have no direct effect on oxytocin (as far
as I understand it). Anything that helps empower mom and boosts her self confidence,
anything that lets her relax and enjoy her baby, will get her oxytocin going.

I think it's helpful when we first start these galactogogues that we spell it out
that these are just to make it quicker and easier to increase, but that her body is
fully capable of this with or without these meds. If everything we say is aimed at
empowering moms, it helps immensely. Everytime we make ourselves and our
interventions and our special technologies (SNS, finger feeding, even pumps)
important, we are disempowering women.  Tina Smillie
