I've always understood that suckling and therefore milk removal is most
effective where the lower jaw (chin) aims with the upper jaw (nose) second.
The areas the cheeks aim at get the least stimulation.  Plugged ducts seem
to occur more in areas where it is easier to aim the nose than the chin,
hence this recommendation.  It makes sense to tell a mom "Aim the chin
toward the plug if possible.  If that is too awkward, then aim the nose in
that direction."
