Thanks so much to all of you who provided me references, etc.  The mom 
I was working with has a history of infant loss and traumatic birth as 
well as low milk, so there was much to be careful of.  This baby, she 
had a birth that was at least acceptable to her, and the baby is 
gaining at the upper end of normal!

Mom said she had sudden, very noticeable breast changes at 38 weeks in 
this pregnancy.  Seems to have done the trick, as she did not continue 
with metformin after all.
