Thank you, everyone, for the warm welcome back and your responses to my question.
I think I could have be a little more specific re: the post.
This babe was 48 hrs old. He had “sounded stuffed up” since delivery (reported when
I first saw him). Unable to stay latched at the breast. Resp rate 70-80. Pulse Ox
94-96% at rest but 88-91% with deep substernal & intercostals retractions with
crying.. CXR, labs, were within normals, but MD decided on Amp & Gent IV “just to
be sure..”
Babe maintained 97-99% Pulse Ox with O2 at 30%. We tried BF after 6 hrs with drop
of POx to 90%. We did have Mom snuggle with babe “skin to skin” (with the Oxyhood
on) for extended periods, which helped Mom to cope.
The next AM CXR showed sm infiltrate Mom had been pumping and milk was in. Babe
could not master suckling/swallowing at breast . He latched, attempted, came off,
cried, de-saturated. Babe still sounded snuffy with upper airway noise per chest
auscultation. Finally, after about 30 hours of this, Dr tried “little noses”
drops. (Neosynephrine nasal drops) and babe began to breathe easier, weaned O2, and
put him to breast with resp. mid 60’s and he latched on, and took right off! He has
had no more problems and is home and reportedly nursing like a champ. Perfect
ending!
Not feeding this particular baby seemed to be the correct choice while allowing him
time to adjust to extra uterine life.
Again thank you, all. I will cont’ to look for the source from a conference and
report what was actually told on to LN.
