Again posting with a clients permission.  This mom is breastfeeding her third baby.  She has inverted nipples which retract after nursing  each time.  I have checked her babies latch several times and it appears to be good.  She has had severe nipple damage from a shallow latch in the beginning, but her nipples are healing now and nipple pain is resolved after the first 30-45 sec. on the breast.  Her problem is burning deep in the breast and up into the axilla after about 10 min on the breast.  The burning becomes more severe about 10- 15 min after the baby is removed and can continue an hour or more.  It is so severe that she needs to use prescription pain killers at times.  It Seems to be much worse at night.  We treated her with Nystatin Cream and her Baby with Nystatin, and she has been on Diflucan 100mg X2 the first day and 100mg a day for 8 days so far with little relief.  She has no temp or soreness in her breast other than the radiating burning.  I'm stumped and am hoping someone might have some other ideas as the cause of the pain.  Her physician is very open to suggestion.
