When the baby can tolerate a bottle of milk it makes me wonder about the
foremilk/hindmilk thing- the milk would be well mixed in a bottle.

 One thing that I have found effective in cases like this is to have mom
express some 'foremilk ' - usually 2-3 minutes or 1/2 -1 oz before latching
baby on. She can even pump simply until the first forceful letdown has
finished. She can do this for a few days, gradually reducing the amount
pumped and discarded. The other measures that you describe are also used.
