I agree with Pat on this one, but I'll mention one other possibility that I
saw years ago when I didn't know much.

It was a mother of twins who literally writhed on the bed when the babies
latched.  Slices at the base of each nipple...  and when she packed her
breasts back in her bra, her long-ish nipples *folded over* right at those
creases.  All I knew to do was suggest she leave her bra flaps down, and the
sores healed right up.  Had it been thrush, the fix wouldn't have been that
simple.

Might this mother have a new bra that folds her nipples over and prevents
them from drying at the crease?
