I sat the exam for the first time a couple of years ago. I had recently completed a lactation-specific course (in Australia) so knowledge & conversations with the tutor were very fresh in my mind.   I also purchased Linda Smith's Exam Review, which I found extremely helpful, which includes written & online components.   I started several  weeks before the exam, testing myself on questions, and then continued to test up to a few days before (& then stopped to focus on other things), gradually pulling up my score. I think I began at getting about 65% correct, and when I finished I reached around 90%.  I would do more reading & note taking on the specific areas where I repeatedly had incorrect answers.For me, getting the marks up meant really reading the question carefully, and only concentrating on what it was asking. Sounds simplistic, but I tried not to read any more into the question than the information that was given and to only choose the response that (by process of elimination) seemed the most correct. I know exam nervousness affects some people a lot more than others, but I have heard said (by IBCLCs who are re-certifying) that the fact that you are already an IBCLC makes it much easier as you have the added bonus of professional experience.
