early bolus of colostrum while it's there to be gotten, and thus maybe stave
off the use of formula later in the day..."  

 

In my experience, and I've worked in a hospital setting for 25 years,
(although just 8 being an LC) that bolus of colostrum does not "disappear"
after a certain amount of time.  I have had moms unable to pump until 12-24
hours after birth and still get that bolus when they finally pump.  Do some
of you have evidence that it gets "reabsorbed" or something after a certain
amount of time?

Also, I don't totally agree that we can assume that there is going to be a
problem with the baby latching if he doesn't latch in that first hour, even
if it is a medicated birth.  Yes, there probably is a higher chance that
there could be some difficulties, but in the majority of cases that I see
(and we have 4200+ deliveries a year), the problems are worked through by 24
hours.  I give moms and (full-term) babies 24 hours before introducing that
pump.
