I am not sure if she is in a skilled nursing facility or not, but if she was on a special bed that "sandwiches" her she would be turned to her abdomen on a routine time frame and when she was turned in that position she could pump.  I cant imagine anyone having to lay flat without the use of this bed.  It relieves pressure to prevent sores etc.
