Hello Wise Lactnetters,
I have two questions that I need wisdom on.

First, for me. I have a 16 month old boy, around 20lbs, has always nursed
like a champ! Teeth started comming in around 4 months. It hurt and there
was a dull soreness that never really seemed to go away, but it didn't
bother me enough for me to try to fix it. The soreness seemed to come and
go though out the first year after that put it was Never severe.
Fastforward to now, with him at 16 months old, and I am in SO MUCH PAIN
that the boy who I wanted to nurse until AT LEAST two may not get it for
the rest of this year. I am to the point of almost crying with every latch
and now understand mothers with intense nipple pain at the start.
The skin does not appear to be broken (I am African-American and the
nipples are dark), but I am sure it is his teeth rubbing me raw. The
nipples hurt mostly on the bottom but he does leave upper teeth marks at
nearly every feeding. I think he has managed to pull his tounge back just
behind his teeth and still suckle at feedings. Is this possible? When I
latch him (or he latches himself) I look to see if his tounge is down. It
is down but not over his bottom teeth. Could he have a short frenulum? I
suspected that when he was a newborn, but the initial soreness seemed to
go away and I just thought he would "grow out of it". Did he somehow learn
to nurse but have his tounge pulled back and now I am suffering the
consequences of it? He is not biting or clamping at the end of a feeding.
He does that too, but this is from the begining to the end and happens no
matter what. Any ideas on how to help? Should I go see an IBCLC in my own
community? My business partner was stumped and I am too.

Second problem: I have permission to post. Mother pregnant with baby #4.
Has nursed all of her children. All have had some type of jaundice and she
says her milk does not come in for several days with two of her children
having to be rehospitalized for dehydration or high billirubin(no wet
dipes in 12 hours)and she started Fenugreek at week 8 with her first and
it seemed to boost supply. She nurses on demand, is a laid back mom and
has not had any breast surgery. She wants to know what she can do this
time to avoid both jaundice and low milk supply early on. We (my fellow
CLE business partner) have suggested of course nursing on the high end of
the suggested times -10 to 12 times in 24 hours, no drugs or pitocin
during labor if possible, drinking and eating well after delivery,
drinking a lactation type tea several times a day after delivery, and
doing skin to skin contact with no separation of baby and mom and seeing
the lactation consultant as many times as possible during her hospital
stay. Should she have fenugreek and blessed thissle on hand "just in case"
since it worked for her before? Is there anything else she can do to
really ensure that she can battle jaundice and low milk transfer from the
beginning?

I would appreciate imput on either or both situations emailed to me
directly or posted to the list - which ever you desire.
