I too have always had a problem with the advice of nurse the baby first.  I
tandem nursed my first set for 3 years and am now tandem nursing my second
set for 10 months so far with no signs of weaning soon by anyone.  Both
times the "tandem" baby was born, the older child went on a no solids binge
for 3-6 weeks.  It was much too complicated to keep track of who nursed
first, last, left, right etc.  I just let whoever wanted to nurse latch on
whatever was available at the time!  All three of my children gained well.
I think this is another example of too many rules making breastfeeding
difficult.
