What an excellent opportunity you have!  I'm jealous.  :)  In addition to other things you cover, I'd suggest including:
* be mindful that bottle-feeding is different than breastfeeding
* when using bottles or pacifiers as tools to improve sucking function and coordination, the goal will be to achieve what is necessary for successful *breast*feeding (not just feeding in and of itself)
* encourage mom to also be working with an IBCLC to protect her supply while the SLP addresses the mouth
* that IBCLCs are trained to know how much breastmilk to use, and how often, and that it's safe
* be mindful of mouth placement on the breast and choose a nipple shape that approximates this
* there is no standard flow for slow flow bottles
* you cannot tip the bottle over to see how fast it flows, as dripping is different than flow
* many are unaware of nipple shields and tubing at the breast as tools to improve the suck and trasfer back to breastfeeding--show samples, instructions on how they work , and where to get them
