I have a couple thoughts.  One is that the taste of the left breast might be
part of it.  Maybe rinsing immediately before nursing, or even trying to
transfer the little bit of saliva and milk that remains on the right breast,
when the baby unlatches.  This would have to be done with a very clean hand,
to avoid adding some other flavor or scent.
This is one of those things that may sound far-fetched, but couldn't hurt to
try!

I also wonder if she has tried offering the left breast while lying on her
right side, just leaning over farther.  It might help to have a small pillow
to put under the mother's abdomen, to help support her and also avoid
putting too much pressure on the right breast.  Elevating the baby a bit, on
a folded blanket or something, might help too.  There was a good description
of this position in an issue of New Beginnings from about 1992.
