I use, in Canada, an ointment that combines antibiotics, nystatin and a
steroid.  I ask the mother to apply it after each feeding and not wash
or wipe it off.  It often works fine.  In the US, something similar may
be had by combining Mycolog ointment with 2% mupirocin ointment.
