This would be for a baby who you know in advance is going to Level III or Level II Nursery immediately with little to no time with mom in L&D. Or you discover at birth that baby is having respiratory or other problems and must be whisked away to the NICU. 
Bring in the pump immediately and have mom pump as much as possible while still in L&D. Baby may not be able to eat a thing for a day or two. Mom's initial bolus of colostrum pumped in L&D can be just enough to feed babe his 1ml, 2ml, 3ml feeds until she can again get more results with the pump to handle the 5ml or 10ml feeds until babe can go to breast. That way, babe may not have to get any formula.
