I find it interesting that Inez gets better insurance coverage when Lactation services are billed under mom.  I have seen that disallowed as it was considered "patient education".  When I worked at the hospital, all outpatient visits were billed under baby as "feeding difficulty" and we got an order from the baby's doc.  I think feeding difficulty was a good catch all because even if the complaint was "sore nipples" or concerns about pumping, it always came back to feeding the baby in the long run.  Incidentally, I was told we got excellent reimbutsement wuth this approach and the only ones that were refused were when there was a deductable that hadn't been met yet.  I wonder if the difference is just begtween the insurance providers or if there are regional differences.
