It sounds like a few things could be going on. The persistent pain could most certainly be a stubborn case of intraductal yeast...has she taken diflucan? Also, if the pain is starting during feeds (and she is taking motilium) maybe she has a case of over-active MER that is aggravating the yeast during let-down.
My last thought is some possible nerve related injury? Has she every had any surgeries that could be causing some neuropathy- because of the "prickling" sensation you describe...1 of 2 things comes to mind; MER or neuropathy.
