I am in the belated throes of updating my topics for our 6 day  course and 
our cram course, and was going through Larry Gartner's info on  jaundice in the 
new Hale/Hartmann book.  The entire time I was reading it  and his 
recommendations for babies to breastfeed 10-12 times/24 hours starting  in the first day 
-- citing, of course, DeCarvalho's work in 1982, it occurred to  me that for 
the most part, it won't happen UNLESS we change the birth  practices.  Another 
article I was reading cited a study (don't remember  which one now) that said 
that nearly 50% of healthy full term newborns latch  & suckle incorrectly the 
first 24 hours.  I'd believe it.  
 
Breastfeeding is so inextricably intertwined with how the  birth is "managed" 
(or not managed in the case of a normal birth) that we are  fortunate we see 
as many good breastfeeding outcomes as we do.
 
There simply shouldn't be so many of US struggling as hard as  we do to work 
with so many mothers who are struggling so hard to accomplish what  is 
supposed to be a normal part of life.
