Cindi, I can empathise with you in this case.
My plan of action would correspond with yours in many ways & would briefly
be
Continue with the antifungals & the acidophilus
Treat baby prophylactically also with oral antifungals
Consider an antifungal diet  -no sugar, no mushrooms etc for mum.
FEEDING Give baby 1 ounce of hindmilk prior to each breastfeed for 1-2 days
Feed baby with mum lying on her side or prone for all feeds for 4 days, then
for the next 3 -4 days she sits up for alternate day-time feeds only. Day 8
feed baby in any position. ( I have had 2 women who have had to revert to
this plan twice in the past 5-7 years to curb a "dramatic let-down")
Consider a trip (maybe 3-4 in fact) to the cranial osteopath regarding the
reflux.
