I would ask the mom to smell and taste the breast milk before giving it to the baby.  Does it taste sour?  Not terribly common, but some women's milk has an excess of something (I think it is lipase) that causes it to break down very quickly.

I had this problem myself, but for me it only seemed to go bad after it had been frozen.

If that is the problem, the mom can scald the milk before cooling it down, to deactivate that excess something.  This is discussed in the new Breastfeeding Answer Book.
