before calling in CPS, first decide what you think needs to be done - if you
think this mom and baby pair need serious help getting breastfeding working,
or maybe getting the parents to understand the importance of this weight
loss, CPS will not help. If on the other hand you believe this baby needs to
be removed from the home, taken to a hospital and given formula (and
possibly put under state care) then go ahead and call CPS. It is a tough
call, because what might happen could very likely make things much much
worse. This woman might need some serious help - in her home, on her terms.
new moms often have a lot of trouble making a lot of appointments outside
the home - she seems committed enough to breastfeeding, to have gotten a
pump and to have (it seems) not supplemented. She might need "permission" to
supplement with formula, or to wean.

It is a tough call, that is for sure.
