Ok, folks, here's my dilemma. I'm working with a mother with oversupply, but
only on one side...example: able to pump 5 oz. (still not softening it) from
one side and 1.5 oz from the other side. Infant is being treated for reflux
and is very fussy and antsy while breastfeeding. Some of her problem, I'm
sure is reflux, but she is also suffering the effects of oversupply. On the
right (over producing breast) baby gets get 70 ml in 5 minutes...much too
fast for her little tummy to tolerate and predominantly foremilk. She latches
shallowly and sucks sluggishly, I'm sure, to control fast flow. I would
ordinarily have a mother with oversupply nurse for a number of feeds on one
side and then switch to the second side. How do I manage this when one side
is slightly under what an infant needs per feeding and the other breast
produces way over what the infant needs. In the past in our center we've had
the mom pump the low side to try to even things out before working on
reducing the supply. In this case my instincts tell me we are not going to
get the "slower" side to increase that much and mother and baby need some
quick relief here. I'm afraid if she nurses 2 or 3 times on the over
producing side she will start to dry up on the other. She could just nurse
on the slower producing side for several feeds and then just express the
other side a minimal amount for relief. Does this make any sense? I'm
anxious to hear your thoughts. I would like to call her with a solution when
I go back to work on Thurs.
