No, I mean that the "production-line" of hospital care is at odds with the 
breastfeeding relationship. A quote from a conference abstract (I was part 
of the research group, someone else will be presenting at the MAINN 
conference in the UK later this year) "breastfeeding doesn’t fit within the 
institutional environment of the hospital because it is unpredictable, 
irregular, time consuming and interferes with patient flow"
There has been research that has found similar problems with birth in 
hospitals.

Re 'core issues' - do you mean for example, individual belief systems of
staff which undermine attempts to implement best practice and 'sacred cows'
- those sorts of issues?
