Susan,
Try to get your hands on "The Breastfeeding Triage Tool" it's put out by
King County Health Dept. in Seattle, WA. I believe the author is Sandra
Jolley...I can't seem to put my hands on it right now.  This book has a nice
checklist in the back and standard questions with responses in a decision
tree. When doing a telephone follow up I modeled my program from the
standards developed by telephone triage programs. Sheila Wheeler MS. has
written a core text on this subject that I would encourage any nurse who
does phone consults to read. I belive it's called Telephone Triage.  Again,
these books are at work.  I'll try to get them for the list.  Many things
must be considered when you develop protocols for telephone calls.
Narrative and checklists are both important part of documentation. WHO,
answers the phone, does the person who gives information have the
appropriate information.  Essentials should include: name of client,
telephone number, age of baby, wt. severity of symptoms, identifying red
flags, automatic referrals etc.  Many things need to be considered in
telephone help.  Great thing to bring to this group
